With regards to the immunization card, do I have to fill out the specific form on the application or can I simply send a copy of my immunization card?

You can either send a copy of your immunization card, submit the MCNY immunization form or have your physician provide proof of vaccination history. We need two proofs of measles, one proof of mumps and one proof of rubella. A combo shot is indicated as MMR in your records. All documentation must be officially stamped. You need at least one proof of MMR in your home country prior to New York City arrival. If you need a 2nd MMR after you arrive, we can arrange that at no cost to you.
